Services
Common Access Card (CAC)
-Appointments must be scheduled no earlier than 60 days before your card's expiration date, unless you are deploying or PCSing to an overseas location.
-Please bring two valid forms of identification to your appointment including one photo ID. All documentation MUST be the originals or certified copies.
-Valid IDs are issued by the state or federal government and can include but are not limited to:
• Driver's license
• Passport (Unexpired)
• Social Security card
• Birth certificate
• Previous CAC/USID
-If you work from home please bring your laptop to log into the base network, which is required before you're able to log onto the VPN.
-Extra appointment slots for future months are made available in the middle of the preceding month.
-We are closed all AFMC Family Days and federal holidays.
-We are closed the first Thursday of every month for training.

Dependent ID Card
• Sponsor MUST be present for this appointment. If sponsor is unavailable, the dependent must provide a hard copy of a DD Form 1172-2 signed by their sponsor.
• Special Power of Attorney is also acceptable to use if sponsor is unavailable.
• Dependents who are 18+ must bring 2 valid forms of identification issued by the state or federal government (original or certified true copy).
• Minimum age of initial ID issuance is 10 years old.
• Dependents ages 0-17 do not require identification if their profile has been previously created in DEERS.
• DoD Civilians and Contractors cannot add dependents to their profile and therefore do not qualify for dependent IDs.
• For Permanently Incapacitated dependents please bring your DFAS determination from https://www.dfas.mil/militarymembers/secondarydependency/sdc/ and a letter from a physician stating the incapacitation.

SrA Dent responded quickly to my question re replacing my Dependent ID that I hadn't realized expired so was taken by gate staff. I had scheduled an appt but wan't until Jan and just wasn't sure if there would be any issues with the lapse in time - he was able to provide a date and time within a week and I was able to get in and get it taken care of. He along with the other staff that were there were very helpful and just appreciate all that you do!!!
