Common Access Card (CAC)
• Lost CACs by military members: a Lost ID Memorandum signed by their First Sergeant/Squadron Commander must be presented. Lost ID Memorandums can be obtained at our front desk or by emailing our org box - wpidcards@us.af.mil

Dependent ID Card
• We can see up to one sponsor and three dependents in a single appointment. If you have more than three dependents, please make multiple appointments.
• Sponsor MUST be present for this appointment. If sponsor is unavailable, the dependent must provide a hard copy of a DD Form 1172-2 signed by their sponsor.
• Special Power of Attorney is also acceptable to use if sponsor is unavailable.
• Dependents who are 18+ must bring 2 valid forms of identification issued by the state or federal government (original or certified true copy).
• Minimum age of initial ID issuance is 10 years old.
• Dependents ages 0-17 do not require identification if their profile has been previously created in DEERS.
• DoD Civilians and Contractors cannot add dependents to their profile and therefore do not qualify for dependent IDs.
• For Permanently Incapacitated dependents please bring your DFAS determination from https://www.dfas.mil/militarymembers/secondarydependency/sdc/ and a letter from a physician stating the incapacitation.

• Full-time college students require a letter from the school registrar certifying undergraduate enrollment in a full-time course of study leading to an associate degree or higher. It MUST have a projected graduation date. You can obtain this from the college registrar office or by going to www.studentclearinghouse.org.
